Earlier this year, the Centers of Medicare and Medicaid Services (CMS) announced $50 billion in new funding for “Rural Health Transformation” grants. While we are waiting for final approvals, it is likely that the State of New Hampshire will receive around $500 million in funding under this program.
This will present potential funding opportunities for certain companies and service providers to serve as vendors to the State of New Hampshire.
The Rural Health Transformation program is aimed at strengthening care delivery across the state’s most underserved communities. The program is designed to support innovative partnerships among a wide array of healthcare, technology, and community entities to expand access to essential services, improve care coordination, address workforce shortages, and modernize rural health infrastructure. With a focus on practical, measurable improvements, the grant seeks to accelerate models that can deliver high-quality, sustainable care in rural regions and improve long-term health outcomes for residents. While the final amount being received is yet to be determined, we expect it to be near $500 million over 5 years.
If you have business interest in any of these areas, defined by the State of New Hampshire as priorities, you should be paying attention to this process.
- Telehealth, digital health, and remote monitoring providers

- Workforce and training delivery
- Behavioral health and SUD treatment

Within the next 2-3 weeks, the State of New Hampshire is expected to begin a process of competitively bidding many services in these segments for rural areas of the state.
